COACHING AGREEMENT

This electronic Registration Form serves as an Agreement between,
I AM Elevating Ministry, LLC., and you (the client). The Services Agreement shall be that of Spiritual Coaching. Please read carefully and contact us with any questions or concerns before signing your agreement. This Agreement will begin on the date agreed upon between Coach & Client, and shall remain valid until the completion of our program.

As a client, I inner stand and agree that I am fully responsible for my physical, mental, and emotional well-being during my coaching sessions, including my choices and decisions. I am aware that I can choose to discontinue coaching at any time.

As a client, I inner stand "coaching" is a Professional-Client relationship/partnership. My Coach (Charrell), will facilitate & support the development of my Spiritual goals. She will also develop and carry out a strategy/plan for achieving those goals.

As a client, I inner stand coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorders or medical diseases.

As a client, I inner stand that coaching is not to be used as a substitute for professional advice by legal, mental, medical or other qualified professionals and will seek independent professional guidance for such matters.
*If Client is currently under the care of a mental health professional, Coach recommends you inform your provider, you have started sessions with us.

As a client, I understand communication is important, I agree to communicate honestly, be open to feedback and assistance, and create the time and energy to participate fully in this program.

SERVICES

This agreement between I AM Elevating Ministry, LLC. and the client, will start on date signed and will continue for THREE months from date of agreement or until terminated by Client or I AM Elevating Ministry, LLC.

During your Transformational Journey, the client will be exposed to tools and techniques for daily Elevation.

We focus on 3 Keys:
1. Un-partnering from low vibrations & negative cycles.
2. Unlocking the Power of The Mind, Tongue, & Heart.
3. Birthing your God given Purpose.

Coach will be available to Client by email and phone in between scheduled sessions as defined by the Coach.

FEES

For your convenience, we have provided payment options below. Please select the best option for you. By selecting an option below, you are agreeing to pay I AM Elevating Ministry, LLC., for the services rendered.

Payment Options for One-on-One Sessions ONLY.

Option ONE

Pay in advance | $450 ($150 discount)
Registration Fee (non-refundable) | $30
If service ends before date, an adjusted refund will be provided.

Option TWO

Pay monthly | $200
Registration Fee (non-refundable) | $30
Payments Due | 3rd of the month
(1) missed payment, service will end until payments are current.

Option THREE

Pay by session | $50
Registration Fee (non-refundable) | $30
Payments Due | 72 hrs. before session start

PLEASE NOTE:
If your journey extends beyond THREE months, the Coach will continue service until the completion of the program at a monthly discounted rate.

CANCELLATION

You must give 24 hours notice if you need to cancel or change the time of an appointment. Otherwise, you will be charged for the session in full. I agree to make every effort to reschedule sessions which are cancelled in a timely manner.

CONFIDENTIALITY

This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality set forth by I AM Elevating Ministry, LLC. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not
subject to the protection of any legally recognized privilege.

The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent.
